For your application to be valid, you must complete Part 7: Declarations. If you under 18, your parent or guardian must complete Part 7A on your behalf as well as Part 7B.

Declaration by Applicant: I certify that the information provided on this Application form is accurate. I authorise the Southern Queensland Institute of TAFE to obtain other details relating to my academic record as necessary. I acknowledge that the provision of incorrect information or documentation, or the withholding of information or documentation relating to my Application may result in the cancellation of my enrolment and the forfeiture of any fees paid, as per the terms of the SQIT Refund Policy.

Declaration by Applicant: Declaration by Parent or Guardian: I have read the information provided on this Application and certify that the details are accurate. I hereby apply for the entry of my child/ward to study with TAFE QLD in Australia and declare that I have the financial capacity to meet the tuition fees and expenses in Australia. I acknowledge that the provision of incorrect information or documentation, or the withholding of information or documentation relating to this Application may result in the cancellation of enrolment and the forfeiture of any fees paid, as per the terms of the SQIT Refund Policy.

Acceptance of Southern Queensland Institute of TAFE International Student Refund Policy and Southern Queensland Institute of TAFE Terms and Conditions.
I have read, understood and agree to the Southern Queensland Institute of TAFE Terms & Conditions and Southern Queensland Institute of TAFE International Student Refund Policy. I understand that this Policy does not remove the right to take further action under Australia's consumer protection laws, and that Southern Queensland Institute of TAFE's dispute resolution processes do not limit student rights to pursue other legal remedies. I hereby undertake to meet the conditions governing my student visa and the by-laws and rules of Southern Queensland Institute of TAFE. I give authority and consent to the Department Immigration and Citizenship (DIAC) to disclose information to Southern Queensland Institute of TAFE of the outcome if I am reported for poor attendance and/or academic failure.
